The Department of Diagnostic & Interventional Radiology offers a full range of outstanding diagnostic services including conventional X-ray, Ultrasound, Computed Tomography (CT) and Magnetic Resonance Imaging (MRI), PET-MRI examinations as well as interventional radiology.

Holding on to the belief that early detection facilitates prompt treatment and hence cure, the Department makes continuous efforts to maintain the highest possible degree of diagnostic accuracy by staying in the forefront of medical imaging technologies.

We always introduce the most advanced equipment, upgrade existing equipment and broaden our scope of services to ensure that patients receive high quality service. Many of our equipment is the first in Hong Kong, rendering our diagnosis fast, accurate and reliable.

The Department is staffed with a team of specialists comprising radiologists, nurses and technicians who have extensive experience in the field. A year-round 24-hour service is provided in a comfortable environment with guaranteed privacy.

Siemens Artis Zeego Multi-Axis System Philips EPIQ 7 and CX50 Ultrasound Scanners
In 2017, Department of Diagnostic & Interventional Radiology has installed two sets of the most advanced diagnostic ultrasound systems to further strengthen Ultrasound diagnostic service. Philips EPIQ 7 and CX50 ultrasound scanners guarantee high efficiency, excellent quality, precise and outstandingly informative examination results for a broad spectrum of ultrasound examinations.

EPIQ 7 Premium ultrasound system has the superb capability of creating ultrasound images of optimal resolution. With PureWave transducer technology, improved penetration and excellent detailed resolution is resulted. It is ideal for the imaging in abdominal, breast, gynecological and obstetrics vascular, superficial structure, thyroid, musculoskeletal system, urology, and paediatric ultrasound examinations.

CX50 General imaging ultrasound system is a compact ultrasound system which allows high quality scanning with easy mobility. Combining PureWave technology to broadband capabilities and XRES adaptive image processing, complete tissue signatures and images of virtually noise-free with superb quality and edge definition can be displayed, bringing a new level of clarity to compact ultrasonography.

In 2016, 3D robotic angiographic system has been equipped in the operating room. The revolutionary angiographic system has an agile eight-axis robotic arm design, which allows it to precisely and instantly reach the imaging position. Surgeons no longer need the OT technicians to manually move mobile X-ray system to image the patient. Fast and easy positioning allow surgeons to check positioning of medical devices whenever necessary. This increases the accuracy of the treatment and shortens the procedure time. In recent years, 3D image-guided operation has proven benefits to improve accuracy and reduce radiation, but its application is limited by the small image size and the sub-optimal image quality produced by traditional mobile X-ray system used in the operating room. There is new opportunity opened by the 3D robotic angiographic system, which scans with unique elliptical orbit and covers large body parts such as the rib cage, the hip, and spine with 8-10 vertebral bodies. The image quality is also profoundly improved by the 16-bit high dynamic range (HDR) 3D processing image chain, with image quality close to CT scan. Patients can be scanned in the operation room instead of being sent to CT suite after the operation. If necessary, surgeons can provide immediate surgical follow up for better treatment results before release, sparing the patient from follow up procedure in another day. The operating room is now in service and serving patients that need to undergo more complicated vascular, orthopedic and spine surgery. These patients can be benefited from better treatment precision, shorter procedure time, and reduced radiation risk.

The new Molecular MR (PET-MR) Imaging Service will be launched at the MRI-PET Centre in Hong Kong Sanatorium & Hospital in September 2015. This service is jointly provided by two imaging departments (Department of Diagnostic & Interventional Radiology and Department of the Nuclear Medicine & Positron Emission Tomography).

The Molecular MR scanner is revolutionary and is the most advanced hybrid scanner in the world. In a single machine, there is full integration of a State-of-the-Art 3-Tesla MRI with a cutting-edge PET scanner to provide simultaneous acquisition of morphological and functional (metabolic) data. This is a milestone in diagnostic imaging that will give new insights into disease processes, and pave the way for new treatment planning and monitoring.

Hong Kong Sanatorium and Hospital is the one of the first medical institutions worldwide to introduce the Siemens SOMATOM Force Ultra-Fast Dual Source CT Scanner. SOMATOM Force is the third generation dual source CT, which incorporates two X-ray tube and two detector systems in the gantry and thus contributes to its ultra fast scan speed and versatile scanning capabilities.

Unlike conventional CT scanners, SOMATOM Force can scan patients at speed up to 737 millimeter per second, at 0.24mm spatial resolution and at routinely low radiation dose and contrast media dose. The ultra-fast scan speed allows whole abdomen scanning in a fraction of 1 second without requiring the patients to hold breath frequently. The low-noise images with best-in-class spatial resolution allow delineation of fine details, giving doctors ever precise information for making accurate diagnoses. Scanning with lower energy X-ray, SOMATOM Force allows CT scan at routinely low radiation dose. For example, the radiation dose of a low-dose Chest CT is equivalent to one Chest X-ray while providing much more information critical to diagnosis. It also requires less contrast media, making it especially beneficial to patients with renal diseases who cannot efficiently excrete the contrast media.

SOMATOM Force is also designed for functional and dynamic studies such as whole heart myocardium perfusion, dynamic GI breed scan and 80cm peripheral vascular scan, as well as dual energy studies for tissue characterisation. Doctors can have a more precise understanding of the diseases before treatment to ensure better procedure success. Patients are spared from repetitive examinations and invasive procedures. It means timely treatment and lower healthcare expenditure.

SOMATOM Definition Flash SOMATOM Definition Flash
Setting new standards for speed and dose reduction, the SOMATOM Definition Flash requires only a fraction of the radiation dose that systems previously required to scan even the tiniest anatomical details faster than ever before. The SOMATOM Definition Flash is a new dual-source CT, featuring two X-ray tubes that simultaneously revolve around the patient's body. The fastest scanning speed in CT (i.e., 43 cm/s) and a temporal resolution of 75 ms, enable for example complete scans of the entire chest region in just 0.6 seconds. Thus, patients are no longer required to hold their breath during the exam the way they had in the past. At the same time, the SOMATOM Definition Flash operates at an extremely reduced radiation dose. For example, a spiral heart scan can be performed with less than 1 millisievert (mSv), whereas the average effective dose required for this purpose usually ranges from 8 mSv to 40 mSv.

This new generation of dual-source CT scanners, featuring two X-ray tubes and two detectors, will provide a genuine innovation boost to dual-source technology. The enormous speed of the SOMATOM Definition Flash offers decisive advantages, especially regarding examinations of moving structures, such as the thorax and the heart. The gantry (i.e., the X-ray detector system surrounding the bore) rotates about its own axis in just 0.28 s. It is this extraordinary rotational speed that enables a scan speed never before attained in CT (i.e., up to 43 cm per second) and temporal resolution of 75 milliseconds. The patient is moved through the CT tube more than twice as fast as with any conventional system. At the same time, scans acquired with the SOMATOM Definition Flash require a much lower radiation dose than conventional scans. While the average effective dose for a heart CT scanner ranges from 8 mSv to 40 mSv, the new CT scanner gets by with less than 1 mSv. In comparison: The X-ray radiation that everyone is exposed to each year from natural sources amounts to 2 mSv to 5 mSv. The dose values of the new CT scanner, thus lie far below those of an intracardiac catheter examination, thus opening up possibilities for using CT scanners for routine cardiological examinations.

Thorax scan in the subsecond range: Breath holds no longer required
Due to the high scanning speed, it is now possible to acquire scans of the thorax, the heart or both at the same time in fractions of a second. For example, thorax examinations now only require a scan time of 0.6 seconds. As a result, patients no longer have to hold their breath during the scan. This offers considerable advantages, especially in cases involving the elderly, children, emergency, and ICU patients.

It is also possible to perform whole-body scans extremely fast: For example, a person with a height of 6 feet 6 inches can be scanned in less than 4.7 seconds. Until now, such whole-body examinations took more than 10 minutes to perform from patient preparation to diagnosis. With the SOMATOM Definition Flash, this process is completed in just a few minutes. This represents an advantage, especially for emergency medicine since, until now, physicians often had to forego this examination method due to time pressure. Furthermore, it is no longer necessary to sedate children prior to the examination, since they no longer have to remain still. The high scanning speed also makes it possible to cover large areas measuring up to 48 cm with 4D imaging (3D plus time). The areas scannable using conventional systems are limited to a maximum of 16 cm due to the detector size involved.

Flexible cardiac examinations with a minimum radiation dose
A scan of the entire heart can be performed in only 250 milliseconds, which is less than half a heart beat. In addition, it is possible for physicians to reliably display a heart with a fast pulse or an irregular heart beat without using beta blockers, thus simplifying the workflow and yielding clinical and financial advantages. Owing to its high scanning speed, the SOMATOM Definition Flash also features new possibilities for performing CT examinations of the heart in the sub-mSv range. This represents a much lower radiation dose than is obtainable with conventional systems, which require doses ranging from 8 mSv to more than 40 mSv. For the first time ever, the heart can thus be examined at a radiation exposure level that is three times lower than the background radiation a person naturally absorbs in a year. The SOMATOM Definition Flash is the only CT scanner on the market that enables the use of non-invasive cardiological diagnostic techniques as routine applications at the lowest possible radiation exposure level.

Double contrast in routine daily work
The second generation of Dual Energy imaging will introduce a new imaging quality. The contrast in CT scans will be increased without having to apply the higher radiation dose previously required. This is achieved via a new, selective photon shield which blocks unnecessary parts of the energy spectrum. It thereby provides improved separation of the two simultaneous scans with low and high photon energy, without causing a higher radiation exposure than would result from an individual, conventional CT examination with only one energy source. Thus, the SOMATOM Definition Flash can always provide a double contrast which, for the first time ever, can also be used to classify the chemical composition of tissues via a CT scan in routine daily work. Subsequently, it could also be used to reconstruct unenhanced CT images without contrast media without having to perform an additional examination.

Dose protection for radiation-sensitive organs
Another technical development for keeping the patient's radiation exposure as low as possible is the X-CARE application. For the first time ever, this application selectively reduces the radiation exposure of dose-sensitive anatomical regions, such as the female breast. This is done by switching the X-ray tube assemblies off during the rotation phase in which the anatomical regions concerned are most directly exposed to radiation. In this way, it is possible to reduce the radiation exposure of individual anatomical regions by up to 40 percent. Furthermore, an adaptive dose shield blocks irrelevant prespiral and postspiral radiation with dynamic diaphragms, thus ensuring that only a minimum and clinically essential radiation exposure occurs. This enables an additional 25 percent reduction of the dose required for routine examinations. The CARE Dose4D software, which analyzes the individual cross-sectional anatomy in real time and adapts the emitted X-ray dose accordingly, also helps reduce radiation exposure.

Being the first high definition CT scanner in the world, the GE Discovery CT750 HD (HDCT) sets a new standard in clarity and dose. The HDCT brings unprecedented image quality across the entire body and reduced dose for better patient care. The revolutionary technology of the HDCT consists of four major elements, namely, Performix HD Tube, Gemstone Detector, Data Acquisition System and Adaptive Statistical Iterative Reconstruction (ASIR). The design of the HDCT brings great improvement in image quality of up to 33% for body scans and up to 47% for cardiac scans compared to previous GE scanners. More importantly, the vastly improved image resolution allows doctors to accurately quantify stenosis in coronary vessels and accurately diagnose problems throughout the body. In terms of radiation dose, the HDCT significantly reduces dose but not at the expense of image quality. The result is an improvement in image quality at 80% lower dose for cardiac scan and 50% - 85% lower dose for all body parts. In short, the high definition breakthrough of the HDCT creates precise treatment paths for diverse patient needs.

In February 2006, a new state-of-the-art BIOGRAPHTM 16 PET/CT Scanner was installed to replace the old PET/CT Scanner. The new system incorporates the highest PET resolution capability and 16-slice CT technology rendering high performance, enhancement of image quality, resolution and throughput.

In addition, with Pico-3D technology, the signal-to-noise ratio is optimized offering over 30% improvement in image resolution. The new system has higher sensitivity that it can pick up tiny lesions as small as 4 mm, as compared to the old system with a sensitivity range of 6.5 mm or above. The scan range of the new scanner is also expanded from 102 cm (or 40 inch) to 178 cm (or 69 inch) to allow PET/CT coscan range for whole-body imaging in one acquisition. The acquisition time is substantially reduced to only 30 minutes for skull-to-toe imaging, whereas the old system requires two acquisitions and a total of 40 minutes for the same imaging. For the first time, the revolutionary matrix coil concept Tim supports all applications in the 3 Tesla range without the need for patient repositioning – from abdominal, orthopedic, cardiological to whole-body examinations at a local measurement field of 50 cm. The core of Tim technology is the revolutionary matrix coil concept. Up to 102 coil elements can be combined with up to 32 radio-frequency channels – providing much higher image acquisition and quality. Tim allows parallel image acquisition along all three axes over the entire body up to a length of 181 cm. A computer-controlled vacuum system that allows directional vacuum-assisted breast biopsy.
After precise localization of the lesion using stereotactic technique, a 11 gauge Mammotome needle is placed inside the lesion. Automatic tissue sampling is then performed. The tissue is first sucked by vacuum into the needle aperture and is then cut by rotational cutter. The tissue cores are delivered to the exterior specimen port for removal without needle withdrawal. Multiple cores are obtained from different sites by rotating the needle aperture within the lesion. When sufficient tissue has been harvested, the Mammotome needle is removed. A radio-opaque marker will be inserted at the biopsy site if the lesion is completely removed by the Mammotome. This facilitates subsequent localization when further biopsy is required. The biopsy wound can be closed without suturing. This Mammotome device can allow accurate tissue sampling, and representative tissue can be obtained for histological assessment.

LORAD Multi-care Stereotactic Breast Biopsy
Coupled with digital spot mammography, the device allows precise targeting of breast lesion in three independent planes. The localization procedure is performed by positioning the patient prone on the table, with her breast immobilized. Digital mammogram is obtained from which the X and Y co-ordinates of the target lesion are calculated. Using ±15 degree images of the target by rotating the X-ray tube, the Z-coordinate is also calculated. Thus the abnormality can be stereotactically localized to allow for the placement of biopsy needle or hook wire into the breast lesion. The available procedures include: Stereotactic fine needle aspiration (FNA), Stereotactic core needle biopsy (CNB), Stereotactic vacuum-assisted CNB, Stereotactic large core breast biopsy using ABBI, and Stereotactic hook wire localization of breast microcalcification.
